An Avatar is a God's physical manifestation on a plane. As Gods are extremely powerful beings, who have their powers spread across the Astral & Material Planes (and possibly others), it is difficult to convalesce their power into a single form. The more powerful the God is, the harder it is to physically manifest.
The Dark God Tarrent used the body of a Dragonborn King as a catalyst to manifest and stay in the Material Realm. It is rumored that this strength was also a weakness, allowing a group of warriors to trap him during the Grand War.
